Problem:  Quartus II software triggers the following Internal Error 
	      in Assembler:
	        Internal Error: Sub-system: ASMIO, 
	        File: /quartus/comp/asmio/asmio_pin_model.cpp, Line: 1728
	        scan_chain_output->has_fanout_less_than(2)


Solution: Quartus II incorrectly connects the phase select signal on
	      clock dividers which have been inserted to connect the clock
	      divider chain. This patch corrects the behavior.


Caution - You must either have previously installed the Quartus II 
          software version 9.1 SP2 or must install the Quartus II 
          software version 9.1 SP2 before installing this patch. 
          Otherwise, the patch will not be installed correctly and 
          the Quartus II software will not run properly.
